Programs that are receiving funding from PCADV must submit Quarterly Reports and other information. These requirements are included in the Requet for Proposal and contracts for pass-through grants and reviewed during orientation session.
After one-year of CBIM implementation, those programs not funded by PCADV may choose to submit responses to the CBIM Implementation Impact Form. To participate, please complete and submit the form below.
PCADV utilizes the information in this form to support additional efforts, help secure funding and/or in public-facing materials. Some responses may help PCADV staff identify potential technical assistance opportunities.
